Session Fees and Insurance

I recommend that you verify your benefits before reaching out for an appointment. While we provide that assistance, you will get the most up to date information by calling the phone number on the back of your insurance card. If your insurance plan does not cover the cost of our sessions either due to a deductible or any other reason, you are responsible for the unpaid amount of my contracted rate with your plan. You may have Out-of-Network benefits. I can provide a superbill to be submitted to your insurance for reimbursement.

Other billable services such as medical records request, treatment summaries, and court actions are not covered under insurance and will be subject to fees listed in the informed consent.

Questions to ask when calling your insurance:

1. What is my copay for an individual psychotherapy office visit, with CPT code 90837?

2. Is prior authorization needed?

3. Is there a deductible? If so, how much of my deductible has already been met? When does my deductible reset every year?

4. Am I limited to how many times I can see my therapist within the year?

For out-of-network reimbursement:

1. How much will I be reimbursed for?

2. What is the process for submitting for reimbursement? What paperwork needs to be completed and where do I send this to?
